Health Requirements

Illinois law requires certain  health exams and immunizations for students to attend school. Completing these requirements on time ensures your child can participate fully in school and sports.

Important: All students must submit required physical and immunization records by October 15 (or within 30 days of enrollment for transfer students). Students who do not meet these requirements and do not have an approved medical or religious exemption will be excluded from school until the required documentation is received.

Requirements by Grade Level

Kindergarten
  • Physical Exam by a licensed health care provider within 1 year of first day of school
  • Up-to-date Immunization Record
  • Parent portion of physical form completed and signed
  • Eye Exam by a licensed optometrist or physician within 1 year
  • Dental Exam by licensed dentist within 18 months prior to submission

Physical/Immunization: October 15 (or within 30 days of enrollment for transfer students)

Eye Exam: October 15

Dental Exam: May 15

2nd Grade
  • Dental Exam by licensed dentist within 18 months prior to submission
Dental Exam: May 15
6th Grade
  • Physical Exam by a licensed health care provider within 1 year of first day of school
  • Up-to-date Immunization Record including Tdap and meningococcal (MCV4)
  • Parent portion of physical form completed and signed
  • Dental Exam by licensed dentist within 18 months prior to submission

Physical/Immunization: October 15 (or within 30 days of enrollment for transfer students)

Dental Exam: May 15

Sports Participation (All Grades)
  • If your child plans to participate in school sports, a valid sports physical must be on file. Sixth-grade physicals may be used if the sports section is completed. Sports physicals must remain valid through the end of the sports season.
Sport Physical: First day of try out or practice for sport

 

Students New to Illinois

If your child is transferring to District 76 from out-of-state or another country, the requirements are:

  1. Physical Exam: A Physical Exam is required regardless of grade level by October 15 or within 30 days of enrollment. It must be completed within one year of the first day of school.
  2. Vision Exam: All students enrolling in an Illinois school for the first time must have a Vision Exam by a licensed optometrist, ophthalmologist, or physician who performs eye exams.

Immunization Records: Translated Immunization Records must show the student is up to date on all required vaccines based on age and grade level.

Frequently Asked Questions

  • When does the exam need to be dated? For the 2026-2027 school year, any Physical Exam dated on or after August 1, 2025, is valid.
  • Does my child need a lead screening? Lead screenings (or a risk assessment by a doctor) are required for all students entering Preschool and Kindergarten.

What if we can’t get an appointment until after October 15? Please contact your school nurse immediately. We can often accept proof of a scheduled appointment (e.g., an appointment card or a note from the doctor’s office) to avoid immediate exclusi
