Medical Resources and Support
We are committed to supporting the "Whole Child." This section provides resources for managing chronic health conditions and finding local medical, dental, and mental health support within Lake County.
Specialized Healthcare Plans
If your child has a chronic health condition, Illinois law requires specific "Action Plans" to be updated annually. These plans provide the school nurse and teachers with clear instructions for daily care and emergency response.

Required if your child uses an inhaler at school.

Required for students with life-threatening food, insect, or medication allergies.

Home/Hospital Instruction
Students may need to be educated temporarily away from the school building due to a medical condition. When a student needs to be away from school for a minimum of two or more consecutive weeks or ongoing intermittent absences totaling 10 or more school days during the school year, the student may be eligible for instruction at home or in a hospital by a qualified teacher. A Medical Certification for Home/Hospital Instruction form must be completed by the student’s licensed physician, physician assistant, or advanced practice registered nurse and submitted to the health office. Upon receipt of medical certification, the District will provide home/hospital services for an eligible student within 5 school days of receiving the certification. If you are unable to see your primary pediatrician, the following local clinics offer school-required exams and vaccinations:

Dental and Vision Resources
- Lake County Dental Program: Offers reduced-fee dental services for children on Medicaid or without insurance. Call 847-377-8800 for an appointment at the Mid-Lakes Center (Round Lake).
- Mundelein Lions Club: May provide assistance for students needing help with the cost of vision exams or eyeglasses. Contact your school nurse for a referral.
Mental Health and Crisis Support
Student wellness includes emotional and mental health. If you or your child are in need of immediate support:
